I've never seen Clapton use a capo on this. It's in D, so if these are noting the chord shapes, they're pretty correct, but Clapton plays it in standard tuning with no capo, so it's in D. The main part of the song is:
D D/F D/E D/Eb
and that kind of descending pattern is typical of Clapton (Tales of Brave Ulysses, Can't Find My Way Home). It works in D but less so in other keys.
