This is mostly right with just a few corrections. The intro is: G Bm (not Cm).
Also, the last chord in the first line of the chorus is G, not D (on the word "sky".) And when the last line of the chorus repeats, "And everywhere I am, there you'll be", the chord changes. It's not Am, but perhaps Cm? I think I hear an Eb in there, but I'm not exactly sure. The first time through it is Am, but when it repeats, the chord is different. I also agree with other comments that there can be some passing tones added between chords. Thanks for posting this!
