Here are a couple things I might change in the tab:

Intro: G C Em D (not G D Em C)
The other way sounds fine, but this is the way my church plays the intro and to me it sounds a bit better.

Verse line 4 & 5:
Em C (not D)
Has been fashioned by Your hand.
G C (not D)
All creation holds together

Chorus line 2 & 3
C G (not D)
You're the Lord of ev'ry man;
Em C (not G) D G C Em D
And Your cry of love rings out across the lands.

The rest is perfect :)
