Your G should be (32003x) other wise it's a GM7. The D5/F# should just be a D/F# because the chord is still a full D chord, the third (F#) is just in the bass (however there is no D/F# in this song the bass sticks to the root the whole song). Also, there is no CM7 in the song. The intro is just the same chords as the verse and chorus and the bridge should be C - D - Em each time instead of D - C - Em the first time. What you have is a good start. Keep at it :)
