Is the whole song really on the same chord :D?

Pretty much ;-)

Using an open E on the seventh fret all you need do is change the root/bass note of the chord i.e. E/B takes it's root from the B you get from the E string seventh fret and E/C# get its root from the ninth fret on the E string.
