Concerning the A# or Bb topic - here's what music-theory says: If you consider this song in d-minor key it's always Bb. Since the chorus is in d-minor, I'd say, the song is in d-minor.

I stick in an extra C chord between the Dm and the A# in the first line and third line of the chorus -- it seems right there. (I just play a C -- the C/A chord sounds funny to me for some reason.) Also, just in case anyone needs the help, Em7b5 is xx2333.
