I've no idea if these chords are how this song is played (I've sort of forgotten it's so long since I heard it). However, they sound lovely when it's all put together.I am going to transpose it into C Major as this suits my voice better and I hope it doesn't loose any of the quality of sound by so doing.Thank you for the hard work of writing out and posting.

pro tip: tune the guitar down one full tone or transpose to -2 and it sounds exactly how phil recorded it,.

This is excellent - just as I remember Phil's own version. We seem to forget our own "fortune" far too easily today , and how easily it can be taken away. (I ried it in C as well - G seems the more expressive key.
