At the end of each line you slide a Bbmaj7 up to a Cmaj7 and back down again, and walk into Cadd9 before returning to D.

The chords are wrong over 'What have I done'. Try C, G, A7sus4, A7.

Oh, and the C in all the verses is better as Cadd9 I think.
