Nice job. One of my favorite Rod Stewart tunes.

For me capo on 2nd fref

The 'C' in the chorus is on the word "you" .... and why the chord position changes in the outro? They're all the same; Em on look; C on reason and D on believe. Some of the other chord positions are wrong also, but you'll have to work them out yourselves.
