The F# should be F#m i think... and yes I don't think the end part is right. It sounds like there are a lot more chords than just the three repeating. Overall, good job though :)

I think the last part might be: (Bm) Don't be scared to (A)speak, don't speak with someone's (G)tooth, don't bargain when you're (F#m)weak, don't take that sharp (Em)abuse. Some patients can't be (D)saved, but that burden's not on (A)you (A7) (A)

I messed around with the chords a bit and I think I figured out the end. The chords for the instrumental after the last "letting people in" are, I believe: Bm A G Bm Em D A Bm Then, starting at "Don't be scared to speak...": Bm D G Bm Em D A
