This is pretty good.
But after the key change it should be "B B/D#" (same as "A, A/C#" just a tone higher)
Don't worry about the B/A#- it's caused by the descending bassline.
If you really want to follow the bassline then play the B/G#m as a G#m7 (which is another way of writing the same thing!)
