you have it mixed around at the end of the verses. The D goes about "quietly we fade" instead of "of our eyes".

I play the bridge (with the hoo hoos) like this, with the number of beats in parentheses:

G(8)--Em(4)--D(4)--C(8)--G(8)
G(8)--Em(4)--D(4)--C(8)--G(8)

Then it gets back to the G-C-G pattern, but I've got it going three times instead of two.

Then it goes back to the bridge pattern for the end, except the last C chord gets 16 beats before ending on G:
