One chord is wrong. There is no C chord, it's an Em, like this:

G B7 [Em] A D

It's also essential to walk up to the G chord. I'm sure most players will recognize this, but for any beginners / country music virgins out there simply play the thick E string 0 - 1 - 2 - 3 and then start strumming yer G.
