I worked out my own version very similar to yours, for the verse instead of a D, i slide the C up two frets and instead of the B/D I play a very similar chord but it's more of a B7, I also tend to play a normal B in the chorus instead of the B/D and instead of my B7, no ieda why, but cracking version mate, well done.

Replacing the D chord: 054030

Yeah it is B7 instead of B/D. Its B7 for the last two bits aswell , the slow bits, instead of D.
