I was going to post this tab, you beat me to it. I play it the same way except for the "come together" bit, where I play D C#m Bm A, which is just following the bass - maybe a little obvious. I won't call it a correction because I'm not sure which is correct, but I think A transitions back to the G a little better than Bm, UNLESS the A Bm in yours corresponds with the last two syllables before the G comes back in, which would be "and"(A) "un"(Bm), which does sound correct.
