A couple of notes:
 
1. the 'slides' I've notated are technically still picked as separate notes, but I highly recommend using
the same finger to play it as the previous note, hence the 'slide'.
 
2. When playing the two final chords, Pat Dimitri seems to mute the notes on the D string. I found this
naturally happened while I was playing, but I've put the actual notes in, in case anyone prefers it.
 
3. I recommend using the normal chord shapes for the first 3 chords- no point in trying to isolate the
notes you're playing, if you hit the wrong string at least it's still the right chord.
