These are the wrong chords. You can tell when you play them on the piano. I transposed this down and figured out the right chords. The intro is B, B/A#, G#m, B/A#. The first line "As I lay me down, heaven hear me now" is B, F#/A#, G#m7, B/D#. Next line "I'm lost without a cause after giving it my all" is C#m7, B/D#, E, F#. Repeat that whole chord sequence for the second half of the first verse. Repeat the whole chord sequence again for the chorus. Bridge "my levies are broken" is F#; second line is E, G#m; third line is F, E; fourth line "I need you to set me free" is C#m7; fifth line is B/D#, last bridge line is E, F#. Repeat chorus.
