The verses and bridge are right. I think the chorus sounds more like:

The chords are good until the chorus. I think where you have D, G Bm, and A, should be G, D, and A. The Bm might go in there also but I am fairly new at the guitar. But if you play with the CD the chorus doesn't match at all the way you have it transposed.
